Our Services

Maternal Mortality is on the rise, particularly in women of color. Moms Matter was created to address this concerning statistic. The program was designed to help prenatal and postpartum women who are managing hypertension and diabetes. Our Nurses are here to provide health supervision, education, and support. This unique program individualizes services to meet the needs of each new mother while providing a seamless continuum of care from hospital to home. Whether you prefer in-home visits, virtual visits, texts, calls, or a combination, we're here to meet you where you are by providing services in any way that works best for your busy life.

The Process:

Participants are enrolled in the program before being discharged. Each participant will receive a blood pressure cuff, and begin receiving calls from their Moms Matter nurse within two days of their return home. Each nurse is paired with a new mother to provide an array of services including health assessments, support, and provision of needed resources and referrals.
